Ozempic vs Wegovy doses

Both pens contain semaglutide, so a milligram is a milligram. The difference is which doses each pen can deliver and what each is approved for.

Dose steps side by side

Ozempic (type 2 diabetes)Wegovy (weight management)
0.25 mg (starting)0.25 mg (starting)
0.5 mg0.5 mg
1 mg1 mg
—1.7 mg
2 mg (maximum)2.4 mg (maintenance)

Is 2 mg Ozempic the same as 2.4 mg Wegovy?

No. It is the same molecule, but 2 mg is 2 mg and 2.4 mg is 2.4 mg. Ozempic tops out at 2 mg; Wegovy's full weight-management dose is 2.4 mg. There is no Ozempic pen that delivers 2.4 mg.

What a switch usually looks like

Because the drug is identical, prescribers generally move you to the matching Wegovy dose step and continue the schedule. Someone on 1 mg Ozempic usually continues at 1 mg Wegovy, then steps to 1.7 mg and 2.4 mg. Someone on 2 mg Ozempic typically moves to 2.4 mg Wegovy or holds at 1.7 mg first, at the prescriber's call.

Pens vs vials

Both brand pens are dialed in mg, so no unit math is needed. Unit math only matters for compounded vials drawn with a syringe, which is what the semaglutide calculator is for.

Why insurance treats them differently

Ozempic is approved for type 2 diabetes; Wegovy is approved for weight management and cardiovascular risk reduction. Plans cover by approved use, which is why the same molecule can be covered under one name and denied under the other. The cost calculator shows the yearly difference.
